Instructions for user

   1.  User must have the below mentioned information before filling the online form:-

• Birth Defect Form
• Info of the mandatory fields as mentioned in the Birth Defect Form
• Photographs of the defect (image size not more than 200 kb). Click here for resizing image

    2.   Select your district, Institute Type and Institute Name for login.

    3.   Enter the password as provided to you for your facility.

   4.  Please fill the Birth Defect Form.
